FOR EXISTING PATIENTS: If it has been longer than 6 months or you have a new complaint, please book a Re-Eval/New Complaint Assessment .  If you don’t see a suitable time, please join the waitlist.  FOR CUSTOM ORTHOTICS AND CUSTOM KNEE BRACES: Book a Custom Orthotic Insoles Assessment or Custom Knee Brace Assessment to get started.  Insurance Note: Most extended health plans cover part or all of orthotics and custom knee braces with a medical doctor’s prescription dated prior to your assessment.   Product Costs: The cost of custom orthotics and knee braces is separate from the assessment fee. Please review appointment details for pricing.   Direct billing is not available for custom orthotics or knee braces. Payment is due in full at your visit. A receipt will be provided for insurance submission.   CANCELLATION POLICY: Appointments are reserved for you. We require 24 hours’ notice for cancellations or changes. Late cancellations/missed visits are subject to a fee: 50% for the first occurrence, 100% thereafter. Please update reminder preferences to avoid charges.
