FOUNDATION REHAB OFFERS IN-PERSON APPOINTMENTS AS WELL AS ONLINE TELEHEALTH APPOINTMENTS. Please arrive for your appointment on time, and bring along attire that will allow your therapist to properly assess your injured area. Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the therapist’s day that could have been filled by another patient. As such, we require a minimum of 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a cancellation fee of up to 100% of the missed appointment’s cost. Occupational Therapy and Kinesiology services MAY be covered under extended health benefit plans. We are currently unable to offer direct billing for OT & KIN services. Instead, you will be responsible for payment at the time of your appointment. A detailed receipt will be provided, which you can submit to your insurance provider for reimbursement. We recommend reviewing your policy or contacting your provider to confirm your specific coverage. Please consider setting up a credit card on file on your Jane (online booking) profile.
