Prior to your arrival: Patients will be emailed and required to complete an online health history and consent agreement.

Cancellation policy: We require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice will be charged as the following; 1st time - $35.00, 2nd time - 50% appointment fee, 3rd time - 100% appointment fee. All appointments within 1 hour or less notice or do not attend their appointment will be charged 100% of the appointment fee. Thank you for your understanding and respect.

We are unable to bill ICBC or WorkSafeBC directly at this time.

All other insurance policies: Will have to be paid in full at the time of your appointment and directly submitted for reimbursement to your extended health provider.
