Patients who provide less than 48 hours notice, or reschedule their appointment, will be charged a $50 cancellation/reschedule fee. Any appointment cancelled/rescheduled less than 24 hours will be charged full charge
We require ALL patients to have a valid credit card on file in order to book appointments online or over the phone. Credit cards will NOT be charged at the time of booking.
Credit cards may be charged if there is a late cancellation/missed appointment that falls under our cancellation policy.
We do not direct bill, so all visits must be paid in full. Receipts will be provide for you to submit to your insurance.
