New patients must contact us first before booking to confirm we are taking on new patients at this time.
1 ) NEW PATIENTS : You must complete your ONLINE INTAKE FORM prior to your first appointment. This expedites your initial intake so we have more time for your treatment. At the time of your appointment you will also complete and sign a Patient Consent Form for Treatment and a direct billing form (if required).
3 ) CANCELLATION POLICY : 24hr notice is required to change or cancel your appointment except in cases of illness or true and unavoidable emergencies. If you need to cancel within that time or forget about your appointment without giving us 24hrs notice, the credit card on file MAY be charged for the full amount of the appointment.
5 ) We are currently NOT accepting new ICBC patients.
