In light of the rapidly evolving COVID-19 situation, our clinic is taking the following precautions to ensure everyone’s safety:

1. You must wear a mask. (bandana, scarf, non-medical grade masks are OK). We have limited masks for purchase $2.

2. You must sanitize your hands upon arrival at front desk or wash your hands in the restroom.

3. A COVID-19 Risk Assessment Questionnaire must be filled out prior to your appointment.
