To ensure our practitioners can provide you with the full duration of your booked appointment, please complete the intake forms before your scheduled time. If you cannot complete them online, arrive 15 minutes early to fill them out at the clinic.

We require 24-hour notice to cancel or reschedule an appointment. Late cancelation fee of $40 will be added to your account if you cancel in less then 24 hours.

If you cancel less then 2 hours prior to your appointment or do not show to your appointment a full price of the appointment will be added to your account.
