Please ensure you are arriving 5-10 minutes early to your first session with us to fill out the intake paperwork, Please contact the clinic and our administrative team can also email the forms before your appointment. Please also ensure you have your EHB information, and healthcare information at the time of your appointment.

We understand that life is busy, and things may happen that you cannot make your scheduled appointment. However, We do recommend that if you need to cancel your appointment to give us 24 hours notice . If a cancellation is made the same day of your appointment or no show your appointment, a $50 cancellation fee will be added to profile.
