Female Pelvic Health Physiotherapy AssessmentNew patient
« -Please select this option if:
• This is your first pelvic health appointment with us, or
• It has been more than 1 year since your last pelvic health visit.
If it has been over 1 year since your last visit:
• You will need to complete a new Health Questionnaire.
• You will need to sign a new Consent Form.
"Insurance Information"
• A referral or prescription from your physician may be required for reimbursement through your extended health insurance plan.
• Please contact your insurance provider to confirm your coverage.
"About Our Pelvic Floor Physiotherapist"
• Lana Berton is a qualified Pelvic Floor Physiotherapist who has completed extensive training through Pelvic Health Solutions.
"Important Information"
• An internal vaginal examination is performed at each appointment.
• If you have a UTI, bladder infection, or yeast infection, please reschedule your appointment.
• Postpartum appointments should be booked at least 6 weeks after delivery and after follow-up with your healthcare provider.
"Virtual Appointments"
• Virtual pelvic health appointments are available for first visits.
• Please return to the booking menu if you would prefer a virtual appointment.
"Coverage"
• Pelvic health physiotherapy is not covered by OHIP, ODSP, or OW.
• Most extended health plans cover these services under physiotherapy.
