NEW PATIENTS

Digital patient intake forms will be emailed to you once your new patient appointment has been scheduled. Please fill out and SIGN the forms PRIOR to your first appointment . We know your time is valuable. We want to maximize the amount of time you spend with the physiotherapist during your first visit rather than spend time filling out paperwork.

Please wear comfortable clothes. All assessments are one on one with your physiotherapist. During that hour, your physiotherapist will assess your condition and discuss patient oriented goals that will direct your treatment.

We offer direct billing for most insurance companies. Your insurance plan might require that you get a doctor’s referral for physical therapy, which you need to bring on your first appointment. You also have the option of emailing it to us along with any additional reports like MRIs, X-Rays, etc. Also, please take into consideration we cannot guarantee direct billing for each and every patient. It all depends on the type of benefit plan you have with your insurance company.
