** Initial Injection Consultation is a prerequisite for all of our B12 & vitamin D Injections** Our highly qualified doctors perform a full assessment of your health history, including a relevant physical, to evaluate your health needs and goals in pursuing vitamin injection therapy. This is intended for patients only wanting to pursue only injection therapy. Patients who are interested in nutritional advise, special testing, acupuncture or any other modality in Naturopathic Medicine should book an Initial Naturopathic Consultation. The visit includes the intake and may include B12 or D if diagnostic testing is in place. Further bloodwork may be required to ensure safety and efficacy of treatment and is not included. Our clinic is dedicated to providing the highest standard of care in injection medicine. Please take the time to carefully and accurately complete your Naturopathic Intake Forms. Forms MUST BE COMPLETED within 24 hours of booking your visit or your appointment will be rescheduled.
