IMPORTANT: Prior to booking a Subsequent Visit, all patients must book a New Patient (NP) appointment. The allotted time for the NP appointment is used to complete a comprehensive health history, provide treatment, update patient records and communicate after care instructions as required. CANCELLATION POLICY: We know your time is valuable, and ours is too. Out of respect for our staff and our other clients, we ask that you give us at least 1 business day of notice if you need to cancel an appointment. Missed appointments, or appointments cancelled without 24 hours notice, will incur a fee. EXTENDED BENEFITS: If you’re planning to use extended benefits for your appointment and, for any reason, the claim can’t be processed at the time of your visit, payment will be required in full.
