New clients are required to complete a health history form prior to their first visit. Please arrive on time, as late arrivals may result in a shortened treatment.
We do not accept Manulife insurance, including OTIP and Manion.
We also do not offer direct billing to Green Shield. Payment is due at the time of service. We will provide a receipt for you to submit to your insurance
