All patients new to the clinic or 3 years from their last visit require a New Patient Intake Assessment. This appointment will last 1-1.5 hours. A non-refundable deposit is required. Cancellation and changes must be made with 24 hours notice or the deposit is forfeit. If you have been involved in a recent motor vehicle accident please book the MVA initial appointment. Note our clinic DOES NOT provide any insurance related services for active WCB cases.
