‼️ All Treatment costs are set to $100 plus GST for our deposit with Online Booking. $100 deposit will go towards your treatment. Actual Pricelist can be found on our website . 📋 INTAKE FORMS: To ensure a smooth process and respect for our Injectors’ time, we kindly request that all intake forms be completed at least 24 hours prior to your initial appointment. If your forms remain incomplete and show as “Pending,” we will be unable to proceed with your appointment, and the appointment will be canceled if the forms are not completed within 24 hours. We will happily remind you and notify you if your appointment does get cancelled. 💳 CC ON FILE: Credit card information is required to complete the intake forms and for them to be submitted for review by our injectors. A credit card on file is necessary for our No-Show and Last-Minute Cancellation policy; however, you will not be charged unless you cancel within 24 hours of your appointment or no show. 🅿️ PARKING : Please provide your license plate number upon arrival, and we will validate your parking for you! If you do not provide this information, parking is $4 per hour through Indigo. Indigo Park Canada Inc. will no longer be able to void any notices issued to vehicles parked at NWH properties managed by Indigo if no valid registration or payment transaction is found. the symm label clinic will not responsible for any parking violations that may occur.
