ADHD Assessment FOR ADULTS - PART 1

The ADHD assessment is a collaborative team based evaluation with the Nurse Practitioner (NP) and a Registered Clinical Social Worker (SW). There are three appointments included. The first can be done with the NP or SW for a personalized assessment, review of standardized questionnaires, and data collection. The second appointment must be done with the NP where medical aspects will be reviewed, diagnosis provided, and medication discussed. Based on clinical discretion, medication may be initiated at the second appointment. The third appointment is with the SW for personal and family coaching. INTAKE FORMS MUST BE COMPLETED AT LEAST 5 DAYS PRIOR TO YOUR APPOINTMENT DATE TO AVOID RESCHEDULING.
