Person’s with Disabilities Application Part 3 with Social Worker

The PWD aassessment includes a one hour interview with a Social Worker for Section 3; One hour interview with a Nurse Practitioner for Section 2; File review and assessment scoring. This session is for the completion of the Part 2 medical evaluation. The part 3 appointment with the Social Worker must be completed prior to scheduling this appointment.
