If you are unwell, we ask for you to please wear a mask to your appointment to protect other patients and our health care team. 2. Please ensure to keep your contact, direct billing and account information up to date in your profile. 3. Please complete all intake forms and consents prior to your visit. 4. If you have medical information or reports you wish to share with us, you may bring them to your visit, fax us at (250) 881-1846 or reception can send you a form to request them from the ordering clinic. To protect your privacy, we do not accept emailed medical information. 5. Pediatric patients: Please be aware we require signed consents from BOTH legal parents or guardians. If you are a sole parent or guardian, we require a copy of supportive documentation. Cancellation Policy: Your appointment time is specially reserved for you. We know things happen, if you need to reschedule please call us at (250) 881-1806 more than 24 hours from your appointment time. Please be aware you will be automatically invoiced a missed appointment/late cancellation fee (50% of the scheduled visit fee) for any missed appointments or appointments cancelled with-in 24 hours or your appointment time. Work schedule changes are not accepted as valid reasons to waive this fee.
