Please come prepared to pay in full for your appointment. You will get an invoice to submit to your insurance company on days that we cannot provide direct billing.
Failure to give 24+ hours of notice for a cancellation may result in a charge of the full appointment cost.
If you reschedule 2 or more appointments back to back with less than 48 hours notice, you will be required to confirm future appointments 48 hours prior.
