IMPORTANT: Prior to booking a Subsequent Visit, all patients must book a New Patient (NP) appointment. The allotted time for the NP appointment is used to complete a comprehensive health history, provide treatment, update patient records and communicate after care instructions as required.

CANCELLATION POLICY: We know your time is valuable, and ours is too. Out of respect for our staff and our other clients, we ask that you give us at least 1 business day of notice if you need to cancel an appointment. Missed appointments, or appointments cancelled without 24 hours notice, will incur a fee.

EXTENDED BENEFITS: If you’re planning to use extended benefits for your appointment and, for any reason, the claim can’t be processed at the time of your visit, payment will be required in full. You can then submit the receipt to your insurance provider for reimbursement.
