NEW CLINIC POLICY REGARDING DIRECT BILLING

ALL PATIENTS : Please note, starting August 1, 2026 we will return to providing direct billing to many extended health plans. Until then, all patients will receive an emailed receipt with your therapist’s licensing information to allow for submission for reimbursement to your insurance company when appropriate and in accordance with your policy and your insurance provider’s guidelines.

Updated intake forms have been sent out to patients. The intake form includes medical history for your chart, along some changes in clinic policies and consents. This document also contains the extended insurance plan information and consents required for direct billing. Please complete prior to your next appointment. Thank you!
