This appointment type is for patients who have undergone a Physiotherapy assessment., but have not been in for longer than 6 months, or who require a longer treatment.

Follow Up Extended Pessary Fitting Physiotherapy Appointment with Aly Huang FCAMPT

This appointment is to only be booked after the initial appointment is completed.

Please choose this option if you have been to Avita within the **last 3-4 months** and seeking treatment for the same Physiotherapy injury. If booking WorkSafe, please select from designated WorkSafe Physiotherapy Category.

Follow up Men's Health Appointment 30 minute (to be booked by Practitioner only)

Follow up visit and treatment with advanced orthopedic physiotherapist involving manual therapy, IMS/acupuncture, exercise prescription

Follow up visit and treatment with advanced orthopedic and pelvic floor physiotherapist with extensive post gradate training and clinical experience

Follow-up Pessary Fitting Physiotherapy appointment with Aly Huang FCAMPT

This appointment is for patients that have had an initial concussion appointment and are following up.

ICBC 30 minute Subsequent Physiotherapy visit (Patient pays $8)

This is for 30 minutes for ICBC clients Pelvic Floor Follow up. Patient pays $23.

ICBC 45 minute Pelvic Floor Follow Up. Patient pays $73.

ICBC concussion assessment (cost fully covered by ICBC)

ICBC Concussion/Vertigo Treatment follow-up

ICBC Extended 45 Minute Follow Up Visit. Patient pays $48.00.

ICBC assessment of concussion symptoms. ICBC pays $141 and patient pays $19. The assessment offers a thorough review of your specific difficulties. ICBC INITIAL physiotherapy appointment is fully covered by ICBC. In order to secure your preferred appointment times, we recommend booking 2 additional subsequent appointments after your initial appointment.

Please book this for your FIRST PHYSIOTHERAPY treatment. Some extended health insurers require a referral from a Physician in order to reimburse for this service. If it's been over 1 year since your last appointment with a Physiotherapist, please select an Initial appointment once again. Please verify your coverage prior attending your appointment. Comprehensive pelvic floor assessment with women's health physiotherapist with extensive clinical experience in pregnancy, postpartum, and menopause.

Initial Pessary Fitting Physiotherapy appointment with Aly Huang FCAMPT Note: The pessary device will be purchased at the time of the initial assessment for an additional $150.

Pelvic Floor Follow Up

Pelvic Floor ICBC Initial In order to secure your preferred appointment times, we recommend booking 2 additional subsequent appointments after your initial appointment. Patient pays $16.

Orthotics Note: Please check with your extended plan provider to determine that they cover orthotics supplied by a Physiotherapist. We cannot direct bill your plan, so the total cost will be collected at the time of the appointment and you can then submit the invoice to your extended plan provider.

Custom orthotics by a physiotherapist. NOTE: Orthotics cannot be directly submitted to the Extended Health Plan Providers. Clients must pay up front and submit to their respective plans. Thank you.
