This 30 minute telephone or virtual appointment is to discuss blood test results, lab results, food sensitivity test results, and/or medical reports ordered either from your MD, specialist, or Dr. Carrie ND.

🌸75 minute initial consultation with Dr. Carrie Rongits, ND at the CARESTREAM MOBILE OFFICE parked at 50657 Green Road South, Wainfleet on the pond's edge. During this time, health concerns will be discussed, health goals will be set, tests will be ordered if needed, and physical exam will be performed. All notes and topics discussed will be provided for you! ✅Please fill out your adult intake forms online before your appointment. ❤️NOTE - CHILD FORMS are for ages 10 and under
🌸This is the SECOND VISIT for a new patient. IN-PERSON appointment is one hour long, and will occur in the CARESTREAM SILVER TRAILER AT THE POND'S EDGE IN WAINFLEET. We will discuss your full Treatment Plan. This will include a review of your health goals, a full health assessment, and how we are going to meet these goals with positive changes! Healthy eating, recipes and food information is the foundation, with discussion of relevant treatment options that may include herbal medicine, Chinese medicine, acupuncture, lifestyle changes, hydrotherapy, homeopathy, and lifestyle changes. Lab results will be addressed if relevant. YOU WILL RECEIVE A PRINTED COLOUR COPY TREATMENT BOOKLET at this time, which will be a reference for years to come.
Your prescription for BHRT or THYROID from me says "zero". and a refill is needed! Blood levels must be tested before a prescription is given, so you can contact me ahead of time for the requisition, or have your MD test it for you. For the appointment day, bloodwork has been done and you have the copies ready for us to discuss. We will review symptoms and be sure that your refill is appropriate. This must be in person.
This VIRTUAL appointment is for current patients that reside in ONTARIO. In this appointment we can discuss current and new health concerns, review health goals, discuss lab results, review progress and plan the next steps.
