New patients are asked to arrive 15 minutes early to fill out intake forms or download them from the website. Patients with extended health plans should bring their insurance card to their first visit. If an appointment needs to be cancelled, 24 hours' notice is required to avoid a cancellation fee. If you arrive late, the appointment will end at the scheduled time and the full original cost will be charged. Reminders via text message or email are available upon request, however, not receiving a reminder does not excuse you from the cancellation policy. Payment for missed appointments can be made in person, over the phone, or at the next scheduled appointment.
