This is a one-on-one private vision therapy evaluation if you are experiencing visual symptoms (including: eyestrain, double vision, headaches, intermittent blurry vision).
At your initial appointment the Vision Therapist will conduct conduct a comprehensive oculomotor function assessment. Following the assessment, the Vision Therapist will complete a report indicating deficiencies and the recommended treatment plan.
Please Note: A Vision Therapy Intake Form must be completed prior to the Initial Session. Records of your last eye exam by an eye care provider must be available.
