First Visit : Please complete the online intake form prior to your appointment and arrive five minutes before your scheduled time
Parking : Parking can be found in front of the clinic space. Celina Lai, RMT shares the same space as Connected Collective Wellness. You may see the logo for that in front of the parking
Insurance : We provide direct billing to most insurance companies. Please bring your insurance card and provide the company, policy and plan number on your intake if you would like direct billing. We are not currently accepting any open WCB cases
Cancellation Policy : There is a 24 hour cancellation policy that includes a fee for any late cancels and no shows. Please let us know as soon as possible if you are not able to make it, as we do try and fill the spot
Payment : A card on file is required for appointments as this will be used to charge any late cancel or no shows, as well as for any appointment fees.
