Si la date de votre dernière visite est inférieure à 6 mois , choisir « Traitement chiropratique (ADULTES) » ou «Traitement chiropratique (ENFANTS/ÉTUDIANTS)

If the date of your last visit is greater than 6 months but less than 5 years , choose « REASSESSMENT/NEW CONDITION and treatment (30 min) ».
If the date of your last visit is less than 6 months , choose « Chiropractic treatment (ADULTS) » ou «Chiropractic treatment (CHILDREN/STUDENT) »
