All New Patients must complete our online Health Intake form to avoid congestion in the clinic and to maximize your treatment time. The intake form can be found in your inbox upon completing your booking. Please fill out the form as soon as possible or within 24 hours of booking the appointment.
**BOOKING WITH TIA: **Tia will perform an interview, assessment and create a treatment plan with informed consent based on your main concerns before treatment starts. Tia often works full body./whole body to restore function, relaxation and renewal.
We offer direct billing to most insurers. Many insurers allow RMTs to direct bill, but however some insurers require patients to pay privately and submit receipts to their insurer directly. Some insurers may also require a doctor’s referral. We ask patients to make sure they understand their insurance requirements.

All RMTs at Crosstown Massage Therapy are opted out of ICBC/WCB, which means we ask patients to pay privately and ask patients to submit these receipts to their adjustor. Thank you.
