Chiropractic First Visit (transferring patient)
**All transferring adult patients and students age 11+ please book this!**
For patients of all ages who have been treated by me before but not at this location yet. Please fill out intake form in advance. You will have to sign a new consent form in person. Plan for 40-60 min at the clinic which includes updating your file, any additional tests, and a chiropractic treatment. This will be at no extra charge to you - cost will be the same as a subsequent chiropractic treatment.
