Please book this appointment if this is your FIRST VISIT with Dr. Sarah, ND. This is a 60 minute virtual visit which involves a detailed overall health assessment, review of intake forms, in depth discussion about current health goals and concerns, review of medications and supplements, and assessment of diet and lifestyle to create a personalized treatment plan. During this visit we may discuss ordering relevant blood work or specialty testing to help create your treatment plan. Upon booking you will be emailed an intake form - please ensure it is filled out prior to your first visit to help efficiently address your health concerns. Please specify whether you would prefer a phone or video call in the notes section.
