Before booking in, please make sure your insurance plan allows chiropractors to assess and/or prescribe custom-made orthotics. A 50% down payment is due on the day of assessment, and the balance is due upon pick up. A receipt will be issued to the patient for insurance claim submission - we are not able to direct bill this product/service.
