IT IS MANDATORY THAT YOU RESCHEDULE YOUR APPOINTMENT IF:

You or anyone in your household have signs/symptoms of flu, cold, covid or any illness currently or in the past 14 days.

Our 24hr cancellation policy still remains the same and you will be charged for late cancellations. To avoid being charged for your appointment log into your account page or email to cancel. All missed appointments are charged the full treatment cost. NOTE INSURANCE WILL NOT PAY FOR MISSED APPOINTMENTS!

I do not accept cancellations over the phone.

Attend your appointment time alone. Do not bring children or others with you.

Please remain in your vehicle when you arrive and I will text you to come to the door.

You are required to wear a mask. If you do not have one it will be provided.

You are required to complete a mandatory screening form. This will be emailed for each and every appointment.

We reserve the right to cancel your appointment if you arrive showing ANY symptoms of a cough, cold or flu. If you arrive sick you will be asked to leave and charged for your appointment. This is to ensure the safety of everyone.

SGI AND WCB CLAIMS ARE NOT ACCEPTED AT THE CLINIC

Note: I can not/will not bill private insurance for any SGI/WCB claims as it is not allowed.
