NEW CLIENTS
Please arrive 5-10 minutes before your first appointment so that we can ensure your forms are complete and your direct billing is set up (if applicable).
Direct billing is a courtesy and is not always guaranteed. Treatment time includes changing time unless otherwise stated.
**INTAKE FORMS **
When you book an appointment, you will receive an email with a prompt to complete the intake form(s). If you have not received an intake form, please contact us to send them to you. Existing clients: we need an update on your health history once a year due to professional college requirements.
✖️ CANCELLATION POLICY
We ask that you respect our therapists’ time, so please give us at least 24 hours notice in advance if you wish to cancel or reschedule your appointment. You can do this online as long as it’s still over 24 hours. A cancellation fee of up to the full cost of the original booked appointment may apply otherwise.
💵DIRECT BILLING
We require a completed direct billing authorization form before direct billing. Please make sure that you are set up for direct billing and have a doctor’s prescription if required by your insurance company, in which a new one will be needed every year. All the companies that we currently bill to are list on the intake form. You are responsible for costs not covered by your insurance company for whatever reason.
