*Please keep in mind that If you plan on getting your orthotics reimbursed by your insurance company, you will need an orthotic medical doctor note. Please bring this with you to your appointment for us to have on file.
**Orthotics are $500.00 including tax, and we cannot direct bill to your insurance company. Payment in full is required up front before ordering. The front desk will give you all the paperwork required to submit for reimbursement to your insurance company.
