You Must book an Initial Assessment if this is the first time visiting our office. Subsequent appointments are for patients who have visited before.
Online Intake Forms Filled Out OR Telephone Contact with one of our staff members confirming the appointment.
If either one of these requirements are not filled out before your first visit, your appointment will be cancelled in order to prioritize other patients.
