This option should be selected if you are new to the clinic and do not have a current file open with the clinician. This option allocates up to 60 minutes for a detailed patient history, physical examination, and first treatment.
Forms will be emailed to you within 24 hours of booking. Please fill the forms out as soon as possible so the clinician can review your intake prior to your arrival at the clinic.
Please wear or bring clothes that will allow our practitioner to access the area of concern.
