First time patients : Please make sure to fill out the relevant INTAKE FORM after scheduling an appointment. If you haven’t filled it out before coming in, you will be required to complete and sign the form before receiving treatment. Cancellation policy : Patients who wish to change or cancel their appointment must do so at least 24 hours in advance. Failure to do so within this time frame may result in a charge of the full fee for missed appointments or cancellations. If you are ill and cannot make it to your appointment, please send an email by the morning of your appointment to avoid an administrative fee. Our policy requires a credit card to be on file for booking. You will only be charged for a no-show/late cancellation or for any outstanding amounts after direct billing to your insurance.
