Appointment & Cancellation Policy
A credit card on file is mandatory to secure your appointment. Payments will be processed after insurance claims are submitted (if applicable), and you can select your preferred payment method at the end of your appointment.
Cancellations & Rescheduling:
Cancellations must be made 48 hours in advance to avoid a 50% cancellation fee.
Rescheduling must be done within the same week with 48 hours’ notice.
Same-day reschedules are considered cancellations and subject to the 50% fee.
Online Intake Forms:
Complete your online intake form with credit card info as soon as you book your appointment.
Appointments will be cancelled if the form is not submitted on time.
Intake forms must be updated annually to keep your health history current.
