Blood Draw & Collection Services (Oak patients only) We offer on-site blood draw and specimen collection for Oak patients, including LifeLabs blood draws and allergy collection. Simply bring your requisition form from your Oak provider and we'll take care of the rest — your sample will be sent directly to the lab for processing. Please note that a valid requisition is required to book any collection service. Patients must have requisition prescribed by a Naturopathic doctor at Oak Integrative. **We are not accepting external lab requisitions at this time. ** Eligible for extended health coverage under Naturopath
