• For the assessment and management of multiple or new injuries, multiple areas of the body, or the inclusion of rehabilitative exercise, please choose 'EXTENDED SUBSEQUENT TREATMENT'/REASSESSMENT.
• Please provide a 24 hours notice if you require re-scheduling, otherwise you may be responsible for the full fee of your appointment. •
