Please review the following important information prior to your appointment:
1. This is not a meet-and-greet appointment. Our private vestibular assessment fee will be invoiced at the start of your visit.
2. Cancellations must be made with a minimum of 24 hours’ notice. Late cancellations or missed appointments will be subject to a fee.
3. All intake and consent forms MUST be completed prior to your assessment.
4. Credit Cards on file are not mandatory, but recommended.
5. Please arrive 10 minutes before your scheduled appointment time.
6. We are a scent-free facility. We kindly ask that you avoid wearing scented products.
7. It is the patient’s responsibility to confirm coverage details and any requirements with their insurance provider prior to their appointment.
8. The assessment may take up to 90 minutes.
