During this appointment, I will gather a full health history, including health goals, lifestyle, diet, sleep and stress.
Please bring any recent blood work or reports if you have any as well as a list of current medications.
At the end of the visit, I will give you a plan to start general treatment., although during our follow-up visit, after gathering more information and doing research, a more specific treatment plan will be provided to get to the root cause of your concerns
