This appointment is ONLY for patients who HAVE a REQUISITION from their Naturopathic Doctor and are NOT REQUIRED to FAST prior to bloodwork.
Please complete our BLOOD WORK CONSENT form prior to your appointment. If this form is not completed we cannot draw your blood and your appointment will be given to someone else.
If you are on any THYROID medications, please wait to take them until after your blood draw appointment.
