Please book this appointment if this is your first appointment with us. At this appointment we will take a comprehensive medical history to understand your needs and how we can best help you. Whether you have an injury or want to relax please take a moment to fill out our intake form. Your medical history is very important to us. We want to be sure you are safe during your treatment. We are happy to offer direct billing to all major insurance companies.
