Virtual Intake -Pediatric Occupational Therapy Assessment (ages 0-15)

This first includes a 30-minute virtual call with our Occupational Therapist with the parent/ caregiver to go over concerns and gather information about the child. You will be sent forms to fill out prior to this call. After this call the child will come in for a 60-minute assessment IN CLINIC. This will be based off your concerns brought up in the call. You will be sent an initial assessment summary outlining observations during the assessment, goals, any strategies discussed and next steps.

Most of this first visit is spent figuring out how your day-to-day is being affected — talking through routines, barriers and goals, and looking at how you manage specific tasks — with less time on strategies or equipment ideas and a clear next-step plan to leave with.
