This visit is designed for returning patients who have been away from care for over a year and would benefit from a focused re-assessment. It allows us to reconnect, review any changes in your health, and update your plan to help get you back on track or shift direction as needed.
It can also be used to address a more minor or isolated concern that does not require a full, in-depth review.
If you are a new patient, please note that this visit is not intended for complex or multi-layered concerns. Due to time limitations, we are unable to complete a comprehensive review of your full health history and all systems, which may limit the depth and accuracy of your care. In these cases, a full Initial Assessment is strongly recommended to ensure nothing important is missed.
