Are you a new patient? We look forward to meeting you! Please call the office to book an initial assessment as your first visit requires a longer appointment. We are booking initial assessments into September.
Returning patients:
Has it been a few months since your last visit? If it has been longer than 6 months since you’ve seen Dr. Floyd, please do not book online . We will book you for an update returning visit which will require a longer appointment block. Instead, please call or email to book your returning visit.
Not seeing your preferred date and time? Not all clinic hours are available online. Please add yourself to our waitlist, or call the office, and we will book an appointment as close to your preference as possible.
