In your follow-up assessment, we’ll discuss what prompted this visit and any new concerns you may have. We’ll reflect on the nutrition goals established in our previous session and evaluate your progress. We'll review any changes in your physical activity, sleep patterns, and stress management techniques, along with updates to your medical history, medications, allergies, or weight. You’ll have the opportunity to share any adjustments you’ve made to your eating habits, including new foods and meal patterns, as well as any challenges faced in maintaining healthy habits and the support you've received. Finally, we’ll summarize key findings, make necessary adjustments to your nutrition plan, and schedule your next appointment.
During your initial client assessment, we’ll gather essential details such as your name, date of birth, and contact information. We’ll discuss what led you to seek nutrition counseling, your primary health goals, and any specific dietary changes you wish to make. Together, we’ll review your medical history, current medications, and food allergies and gather measurements like height and weight. We’ll analyze your typical eating patterns, including meals, snacks, and any challenges you face in maintaining healthy habits. Additionally, we’ll explore your physical activity levels, sleep habits, and stress management techniques. Identifying your support system and any health professionals involved in your care is important. Finally, we’ll summarize our findings, discuss initial recommendations, and schedule your follow-up appointment.

Follow-up sessions are where integration, refinement, and progress happen. In these sessions, we: ⦁ Review changes, symptoms, and improvements ⦁ Adjust nutrition, supplements, and lifestyle strategies as needed ⦁ Provide ongoing education and support ⦁ Address new patterns or challenges as they arise ⦁ Continue building body awareness and long-term resilience Healing is not linear—follow-ups allow us to respond to your body’s feedback and move at a pace that feels supportive and empowering.
Initial Assessment: Our first session is a comprehensive, collaborative conversation designed to understand you as a whole person, not just your symptoms. During this assessment, we will explore: • Your main health concerns, goals, & priorities • Health history, medications, supplements, and previous diagnosis • Digestive health, energy levels, sleep, mood, and stress patterns • Dietary habits, food intolerance, cravings, and lifestyle rhythms • Emotional, environmental, and behavioural factors influencing your health This session is both fact-finding and pattern-finding. We connect the dots between what your body is communicating and what it needs to restore balance. By the end of the assessment, you’ll have: • An understanding of what may be contributing to your symptoms • Initial education around root causes • A general roadmap for your personalized care plan Personalized Wellness Plan: Following your Initial assessment, a customized wellness plan is created based on your unique bio-individuality. This may include: • Targeted nutrition guidance and therapeutic food strategies • Supplement recommendations (only if necessary) • Lifestyle support (sleep, stress, routines) • Sustainable changes that fit your real life Plans are practical, flexible and evolving—not a one-size-fits-all.
