Please use this appointment if you have been to our clinic before for Physiotherapy but haven't seen your Physiotherapist for 1 year or more OR if you have been to our clinic before for Physiotherapy but have a new injury that hasn't been assessed by your Physiotherapist yet. This allows your Physiotherapist to both assess and treat your new injury. Please complete the intake form before your visit. All visits are subject to a 24-hour cancellation policy. Missed/rescheduled visits will be charged a cancellation fee in the amount of your visit cost.
