In order to secure your appointment these forms need to be thoroughly completed before your scheduled appointment time. Due to the demand and consideration of other patients who may require an appointment there is a No Show policy in place should you fail to attend or cancel your appointment. The No Show fee is the $125 cost of the Initial Chiropractic Assessment.

After this initial assessment your child future visits should be booked under a Subsequent Chiropractic Treatment. If your child is 5 or older please book an Initial Chiropractic Assessment.

CLICK HERE If you have seen Dr. Bayley in the past but have a new region of complaint please book this type of appointment so that she has appropriate time to assess and treat you. If you have been told by Dr. Bayley that you require a Re-evaluation at your next appointment please select this appointment type as well. (Re-evaluations are required at minimum every 12 visits as regulated by the Alberta College and Association of Chiropractors).

Can be helpful for treatment of: Plantar fasciitis, Achilles Tendinopathy, Trochanteric Bursitis, Tennis/Golfer's Elbow, Calcific Tendinitis of Shoulder, Trigger points (Tension type Headaches), Iliotibial Band Syndrome, Medial Tibial Stress Syndrome (Shin Splints). **Effectiveness of treatment is based on one treatment per week for three consecutive weeks.** If you have insurance coverage for chiropractic care, all or a portion or shockwave therapy may be covered.
